Relax, you're in good hands.
We have some information you will need for your first appointment.
Your initial appointment will often consist of a consultation with the surgeon explaining your diagnosis and treatment options for your oral and maxillofacial surgery. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please Bring With You:
- Your surgical referral slip and any X-rays if applicable.
- A list of medications you are taking.
- Both your medical and dental insurance information.
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (such as diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are taking any medication (such as heart medications, aspirin, anticoagulant therapy, etc.). These details are important in the planning and implementation of your oral and maxillofacial surgery.
Thank you for trusting us with your oral and maxillofacial surgery needs including: wisdom tooth removal, dental implants, tooth implants, dental bone grafting, cosmetic procedures, corrective jaw surgery, sleep apnea treatment, TMJ pain treatment, and cleft lip and palate repair.
